Does anyone have a CLIA Lab Director Site Visit Form or Checklist they are willing to share? Also, does your Medical Director perform the site visit or do they delegate it to another Leader?
We are trying to establish the process here and I would be eternally grateful for any suggestions and/or forms you can share.
