POCT: BUN/Creat fingerstick

Aloha, is anyone familiar with an FDA approved POC test system that provided BUN/Creat results via whole blood fingerstick?  Intended use is within the Imaging Dept for contrast screening.  Any assistance would be greatly appreciated.  Have a great day.

StatSensor Nova Biomedical for creatinine used in Radiology.

Know that the Nova instrument is non-waived and provides just creatinine. The ISTAT is also non waived if it is done off of anything but a green topped tube but will provide BUN/Creat if a CHEM8 cartridge is used.

I believe most POC analyzers that perform Creat/Bun testing are considered moderately complex.  We use the Epoc in our Radiology department and have had good success with it.  Easy to use and compares well with our main laboratory instrumentation.

We just purchased Nova Creat STAT Sensor.  CLIA Waived just like their Glucometer.  Resonable priced.  We are using it in our ED and IRAD departments.

How was your correlations?  We have had the StatStrip on board for almost 5 years now.  

The limiting factor for us to utilize the I-STAT Chem8+ was the need for a venous draw in Imaging.  They would like a system that was less invasive.     

We also use the NOVA Statsensor, and it is non-waived (just confirmed with Nova over the phone). They provide the Creatinine and eGFR calculation and are used in our Imaging and IVIG departments. Be aware that the testing is in the strips, just like in the cartridges for the iSTAT, so you may see differences between strip lots.


Unfortunately, our Statsensors do not correlate well to our AU480 analyzers and Nova's solution is to do a 20 patient correlation with EVERY new strip lot, calculate out an offset factor, change the offset in Novanet, and then another 10 patient correlation to verify the offset. So hopefully your correlations are good, otherwise it becomes a real pain.


They do not sequester strip lots because they are refrigerated so they cannot be manufactured in bulk. Another consideration is how to house the strips and QC in temp-monitored refrigerators in departments.


 

Our Imaging Center use I-STAT.  They draw blood from the IV line that they use to inject the dye. They use an adapter that will draw the blood  from the line into the green top tube.  Then they use SafeKem to inoculate sample into the cartridge.


They already had the I-STAT programmed before they draw the blood so the process takes only  2-1/2 minutes by the time they insert the IV line, get the blood, test on I-STAT and get result. If the results are good, they proced with dye injection.


 The doctor just want the creatinine to check kidney function. .

For the IV line draws are you utilizing RN's?

We also use the EPOC in radiology for Creat. The EPOC doesn't do BUN at this time but they've told me that will be on the next card upgrade later this year or early next year.


 


The techs will draw venous blood from the line they start to test. You can purchase capillary fill tubes for the EPOC if you want to stick with just capillary specimens.


Non-waived, but correlates well to the Vista.
