Possible Heparin Reactivity Issue Causing Discrepant ACT results in Cath Lab

Our cardiologists have told us that the ACT results on the i-STAT are discrepant, but then mentioned that there was also a possible problem with the reactivity of the heparin being used nationwide. Of course, they couldn't tell us which patients were involved. We're having them document the lot number of heparin that is being used, in addition to notifying us when the situation occurs.
Has anyone heard of any problems with heparin reactivity or had any problem with discrepant ACT testing on the i-STAT recently?

We have had the same issue and use the Medtronic ACT Plus. The compliant was they feel like they are pushing a lot more heparin before the see the anticipated response.
We have found that our pharmacy was using different lots of heparin from different countries-India v. China. Also there seemed to be a timing issue of when the patient was given a bolus of heparin and when the ACT was drawn/tested.
Have not heard much else after.

Has anyone had this problem with the Hemochron Sig Elite analyzer? Can someone please send me the study? We are having issues using the ACT-LR cuvette only during vascular procedures. Surgeons not confident in the analyzer.

Thanks to this listserve, we made contact with our pharmacy. They are aware of the heparin shortage potential and was already keeping 4-6 month supply and checked with their supplier to see if they will be having a shortage. They will also let lab know if they change suppliers so we can proactively work with our IR, Dialysis Cathlab and ECMO teams.

We use Hemochron sig elite ACT+ and there have been a two cases now where I have received a complaint that the ACT did not respond as the physician expected to the heparin dosage given . These were 'Impella' patients - a new procedure to our facility.

It is time to replace devices and our affiliated sister hospitals use I-Stat so I leaned toward standardization.
But, I read a lot, talked with other users, and communicated with physicians.
