We are in the early stages of deciding which instrument to use in the ED.


Does anyone have any thoughts or experience with the istat vs epoc vs piccolo?

It would depend on the testing menu.  We use Piccolo for CMPs and I-Stat for B-hCG, blood gases and lactate, and troponin. 


They are looking at the chem 8 at this time.

Does the piccolo have barcode scanning capability or does the end user have to manually enter the user ID and patient ID?

You can plug in any standard barcode scanner which is what we did to scan the patient ID.  But you do still need to manually enter sex, race, and DOB.

The piccolo is a nice little instrument that will give you the results in 20 minutes and it is waived testing. It is user friendly but not really something you want to carry around. If you can use a smart phone, you have the piccolo down. This is an easy platform to use. The cartridges are expensive and need to be refrigerated. The controls must be frozen. 20 minutes can be a long time compared to the other options of epoc and iStat. I cannot speak to iStat. I can speak to the epoc. It is moderate testing. The test cards are room temperature and the controls are refrigerated. The epoc only has gases and the basic chemistry as options. HCG or Troponin are not options. It will need an IQCP. I like the epoc because of a few special features:



  • Room temp storage of cards.

  • Portability of unit.

  • One card for all testing (gas and basic chem) and you can edit what the user sees.

  • If the user injects too much blood, the card is not lost and you should still see results.

  • The epoc connects via wifi to the printer and will print with the press of a button. You do not need to line anything up to print.


There are a few things about the epoc that take time to figure out or is not such a plus



  • injection of sample. It does take some people a few times to get the injection down.

  • We have the epocs connected to Telcor. That is nice but all of the user access is through the epoc data management system, EDM.

  • I have not been particularly happy with the creatinine, specifically. It's just okay when compared to main lab.

  • Hemodilution factor. This is for the measured hematocrit and calculated hemoglobin. If the patient has had more than 1 liter of crystalloid fluid, the user has to check a box that will correct the hematocrit for the fluid.

I have a Piccolo for a containment room.  But it does not correlate well to a Vista.  Do you have any history of comparing it to the Vista?






I use the istat in NICU, Cath labs, Perfusion and Radiology.  Overall it is a nice meter.  Abbott hasn't changed much in the last 20 years.  One thing that held us back from switching to the Epoc was the moderately complex platform.  In Radiology it is a waived
test.

For the Piccolo comparison to main laboratory, we used the Roche 8000. The Piccolo was intended for the emergency preparedness (Ebola) patient. We closed down our response and tier level with the State. It was never used on a patient and is now for sale :)
