I have a surgeon who wants access to our POC INR meter for pre-op patients. Our ACC uses CoagSense for their Coumadin patients. Looking at the insert/user manual: The device is intended for use by properly selected and suitably trained patients or their caregivers on the order of the treating physician to monitor patients who are on anticoagulation therapy.
Patients should be stabilized on warfarin-type (coumarin) anticoagulation therapy prior to self- testing.
The device is not intended to be used for screening purposes.
When I asked if he was wanting to use it with only coumadin patients he said that he wanted it day of surgery to check INR of coumadin patients who had stopped the drug for 4-5 days prior to surgery. Is this within the bounds of waived testing do you think? Our surgery center is not at the same location as the lab or I would just say to send a STAT lab sample. 'Stabilized' has me wondering if the CoagSense is the appropriate device for this setting. If the patients have been off their meds for 4-5 days, wouldn't they not be stabilized? Or am I overthinking this?

I would think if someone is trained on the machine, it would be okay. I am guessing the "stabilized" comment is just for self-testing at home; not a general statement on the use of the machine by professionals.
