Lactic on CG4 Cartridges

Hello All,

We received a notification from South Dakota CLIA that we can no longer perform G3 Cartridges. 
We are looking at switching to CG4 , and looking to see if we block lactic acid or validate it along with the blood gases.
What are everyone's thoughts on the lactic ? Pros and Cons?

I had heard that Abbott was no longer going to offer the G3. Using the CG4 cartridge, you could just turn off the lactate in the DE server, so it would only put out the blood gas results. We do use the cartridge both with and without lactate, and the only issues we have is with the "blue" CG4 cartridge. It cannot be used for capillary samples, and the pH lower range is 7.000, too high for most of our uses. For those sites, we are using the older "white" cartridge, which doesn't have those constraints.

We are using CG4 in our ED.  They do use Lactate, and we do a patient correlation between CG4 and ABL90 (Radiometer).
Our respiratory department performs any capillary blood gases, and the emergency department has access to both Istat (as POC) and respiratory (run by respiratory).

Would you be willing to share the letter? Did they give a time frame for when you have to be off the G3?

We have a similar process as Kathleen. We only use the white CG4 as the pH range for the blue CG4 does not work for our organization. We have test select turned on so they can just run a gas or can include the lactate. We have long used the lactate and have no issues with it.

For those of you who are using the CG4 cartridge - how did you validate the cartridge?  We currently use the ABL90 and I'm not getting good correlation values for the Lactates..  I'm using Li Hep tubes that have been saved from the night before and re-running on both the i-STAT and ABL90.  Any help would be appreciated.

Connie, we drew fresh samples and ran them for comparison and the results correlated pretty well.
