Time "sync" issue with POC devices

We occasionally have issues where a test is uploaded to the EMR prior to it actually being documented as being performed.

Systems involved:
  • Siemens Epoc to POCcelerator to QML to Epic
  • Nova Statstrip/Prime Plus to NovaNet to QML to Epic

example:
  • test is documented as being performed at 12:00:00pm  (hh,mm,ss) on the POC analyzer.
  • uploads into QML at 11:59:05pm
  • is pushed out of QML to Epic at 11:59:45pm and Epic error's it due to it being uploaded prior to its documented test completion time.

I suspect the different platforms are "sync'ing" down to the minute and not the second which is causing this issue when everything lines up perfectly and it uploads very quickly.

Has anyone else come across this?  Solutions?

We have the same problem with our Siemens Clinitek Status Plus.  Even, a software update did not fix the issue.  Interested in any suggestions to solve this problem.

If you are not already using this, or familiar with it, the QML system allows a "grace period" versus being set to 0. As you can see on this Nova glucometer, we allow a 1 hour admit grace time to cover test(s) that may have been run prior to the patient's actual admission time. Or, if using the Clinitek, it allows for time differences between the analyzer time and the HIS clock time.  We even had to adjust for our Endo clinic due to providers wanting the A1c while the parent was getting their child checked in.

We have this issue with our ABL90 clocks going fast and used to see it with our Cliniteks.   If your sending thru QML, create a repeat rule. We hold all ABL90 results in QML for 3 minutes using a repeat rule so that by the time QML sends the result to EPIC its never too early and EPIC doesn't reject them.  Works for us.

Sam,   The issue isn't with QML. I do have a 4 hour grace period for all tests in QML and the tests are being uploaded from QML to EPIC.  The issue once it is uploaded into EPIC it errors it out without posting it in the EMR.

Katie,  I've asked Telcor about holding tests for a couple minutes and told it could be done but it wasn't recommended.  Thanks, we may have to go this route.

Background:  Using different middleware, oversee physician office group but utilize health system middleware for interfacing.

What I have been told from the health system POCT is that it is from EPIC.  If the instrument time is ahead of EPIC (in your example 12:00 instrument, but EPIC is 11:59) it doesn't like it.  We just change the time of the instrument to be on EPIC time or even a little slower.  However in some cases they need to be constantly checked to make sure they aren't ahead.

We have observed this in EPIC as well.  It is not from QML issues.  When we see this, we have staff change the time on the instrument.  This is a common issue that we have with the DCA Vantage as well it runs fast often.

Use the Set Time feature in QML.  This will sync your device with QML.  EPIC will accept the result because the source is synced by QML.
