Rejected results

Does anyone monitor the # of rejected POCT results?  Is it part of your POCT quality measures?  We use the Novastat strip glucose meter, interfaced to RALS, and the GEM 5000 interfaced to GEM web.  The end user can choose to reject results and this is documented in RALS and the GEM web.  I do see where results have been rejected but to be honest I haven't been investigating or monitoring this.

We use the Nova StatStrip glucose meter with RALS.  I have the rejected results set to hold for review in RALS and verify that the results were meant to be rejected.  Often times I see that the operators don't realize that they have selected "Reject", even though a comment must be selected if they do.  I look at results that are rejected, see which comment was selected, and see if a repeat test was performed.  If they select "repeat test" and I am unable to see that a repeat test was performed or if they selected a comment of "provider notified", I send the operator and their supervisor an email asking whether or not the result was truly meant to be rejected or if it was rejected in error.  More often than not, reject is selected in error and I transmit the results into the EMR. 

We use Telcor and rejected results go to a certain section where we have to review them before sending them to storage.  We do scan them to check for odd ID"s that might signal a coworker doing testing on themselves or a coworker and then we will investigate.  We do not review individual rejections most of the time.  We do not report counts in our quality committee metrics.

I agree totally with Ashlee. We have MANY Rejected results with the Nova StatStrip meters, and MOST of them are unintentional. I think the placement of the "REJECT" button on the screen is the problem.....it is often where someone's thumb might rest when holding the meter. I too have it set up to have a comment entered, but often it is "RN Notified"  or "Action Taken", which forces me to have to contact the user and see if it should really be uploaded to a chart. Often they are surprised that they "rejected" it. I feel like I'm educating each operator one at a time! I do monitor it monthly and hope to see an improvement, but sadly I haven't yet!

We do not allow users to accept/reject any test ran on a POC analzyer.  If a user believes there is an issue with the test/results they must submit a POC Correction form to address it and correct the patient's medical record if needed.
  • This allows us to monitor issues, if the user is blocking the results... do you know why?  Is there a systematic issue that needs to be addressed?
  • This also stops users from running tests on themselves which is not allowed.

We do have a similar issue with the StatStrip button placements.  We use MTE (Manual Test Entry) for Dipstick, HCG and Strep on the glucometer.  The buttons for these are close to  each other and on occasion we discover that users enter in HCG tests results as a Strep result (or visa versa) in error.  There isn't a way to move the buttons on the screen to separate them more.
  • This is still an improvement over the older data entry method of requiring the staff to find a computer, login, pull up the patient and manually enter in the results.
