POC result corrections

Good morning, 
I have a question about result corrections. I've had multiple tickets lately of a glucometer or epoc result being sent to the wrong patient chart. 

Are your facilities POC team allowed to remove incorrect results from the chart? Are they then allowed to change and resend the results from your QML/RAALS to the correct patient chart? 

Our policy doesn't clearly spell this out; it just states that any mislabeled specimens in the lab must be recollected or if they are a difficult specimen (CSF, body fluid) the collector must come down to identify and relabel.

Our policy states that POC can remove results from the incorrect chart, but we cannot transfer results to the correct one.

We are the same as Kayla. We can take results out but we do not put results in. 

Essentially these are mis-labeled/mis-identified specimens that have been ran and then documented on the wrong persons medical record.  As such, they are treated the same way as any other mis-labeled specimen the lab receives per policy:
  The results get omitted/credited (note: the test is not deleted) off the wrong person's chart. A comment is placed on the test stating the error made.
  • If the sample is considered *un-recollectable* the results are transferred to the correct chart with a comment stating the error made and the user who re-identified the correct patient.  examples: arterial and cord samples.
  • If the sample is considered recollectable the user is responsible for obtaining a new sample and repeating the test.  examples: urine, venous, swab.

Hiya! We remove results and issue a credit for the test. Glucometer and EPOC are both retrievable samples, unlike your CSF example that's considered irretrievable. 

GM, we require the operator to file a safety event for documentation, then remove/credit erroneous result similar to Candy's response.  Usually, they have already retested correct patient.
