Point of Care PTH Testing

We've been asked to provide point of care PTH testing in surgery.  We provide PTH in the main lab.  What instrument do you use for point of care?  Can you share your experience(s)?

Hi, we recently went live with the Roche cobas e411 in our OR last month.  Initially there was pushback about whether it classified as a POC analyzer due to its size.  It's the only test that we are performing on the analyzer at this time.  So far there have been no issues and test is resulted in about 10mins. We have it set up in a room next to the OR, equipped with a centrifuge and refrigerator for reagents.  The instrument in the main lab that used to run the PTH is the Abbott Alinity but uses green tops.  Cobas uses only lavender tops for PTH.  We couldn't do correlation because of that.  So we opted to compare with a nearby hospital that uses the e411.  The staff hands us the sample, we call into the OR with the result and it is also interfaced to Harvest and Epic. 

Thanks for your quick response!  Do you have laboratory or non-laboratory employees performing the testing?  Do you have the instrument interfaced to the EMR?  

We have the BC DxIs. They have an intra-operative version of their PTH assay that is faster than their normal one. We do the intra-op PTH measurements in the main lab.

Thanks for the reply!  Do you use the intra-operative one, but in your lab instead of in surgery?  What is your TAT?

I use the Future Diagnostics. It is located in a room right outside the OR. They bring the sample to me and I run it…~7 minute turn around time. It is only run by myself (POC Coordinator) and one other lab tech. It’s a very dependable analyzer however we are getting rid of it and bringing it into the lab on the Atellica. It’s not cost effective to keep it in use. I’m pretty excited to get rid of it! iOPTH is really not a point of care test in my opinion. Good luck!

We have a small team of RNs that run the Tosoh AIA360. It works well but does come with a 20 minutes TAT.

We have lab techs performing the test.  We don't have a high volume of ioPTH samples (3 surgeries a week which would equal to about 8-10 samples a week).  Once the case is over, the tech goes back to the main lab to finish out her shift. The analyzer is interfaced. 

Has anyone heard of the NBCL CONNECT instrument?  It looks like they have a whole blood PTH available in Europe.  It doesn't look FDA approved yet?  Their website says they were at the AACC meeting in July 2023.
