POC TEG 6S Sample Comparison Studies

For anyone that has the TEG 6S in the POC - we have 2 of them in CVICU and 2 in CVOR...how are you doing your sample comparisons required by CAP?  Are you comparing to the Lab TEG 6S?  I'm having trouble comparing all 4 instruments to the lab using 1 sample due to stability and quantity, not to mention expense!

We use QC data and have utilized the Haemonetics Validation Analyst to crunch the data and give us a nice neat report. We use the one-way ANOVA criteria. This was a free service to us and not hard to set up, add to our policy for correlations and implement. You should give Haemonetics a call and see how you get started.

We do patient comparisons between 3 TEG6s currently.  It is difficult to get all 3 to be available at a time so we have to watch the CVOR schedule pretty closely for when might be a good time.  We don't get all of them done on the same day.   We usually don't have too much issue getting enough blood as we use blue tops from the Main Lab.  They have TEG5000s currently.  If we can't find enough blood from them, we will draw volunteers from the Lab.

Our lab POCT planning to implement TEG 6s in OR's soon. We plan to use QC data for comparisons. 

Question - how do others track which operator did the test if there's no operator list/lock out and just a generic login?

That is a limitation of TEG, as the software has not really been built to accommodate running in the point of care setting.  We have to use paper logs to document the QC.  I also don't like that the user has to tell the instrument whether or not the QC Passed or Failed.  So for right now, it is a manually data entry on a spreadsheet to look at comparisons

We use the TEG 6s with RALS as the middleware with Cerner using an unsolicited order much like the glucose meters.  RALS will keep up with the operators, the cartridges/cuvettes & the QC.  TEG Mgr will allow the operators to log in and run the tests while it also allows the clinicians to view the results in real time from any work station.  I don't think TEG manager will prevent the operator from logging into the instrument but RALS will warn us about expirations.
