Separate CLIA

Florida question, I have 2 locations that are not in contiguous buildings on my campus that are interested in POC testing. Can I include them in the hospital CLIA or do they need their own separate CLIA certificates?

It depends. It is not as logical as you might think. Check your local CLIA reps. If the locations are considered part of the same larger entity they may have the same CLIA number. I know of one instance where 2 labs under same director, same staff, were situated at forked road (not contiguous buildings) and CLIA advised them to apply for single certificate. CLIA Form CMS 116 for example has a multiple site exception (Section V)
