POC Respiratory Interfaced Devices

Hi there!  We are currently looking into platforms to use at POC for the next respiratory season. We have looked at a few systems. Can you share what you might understand about the Roche Liat, Biomerieux Spotfire and the Cepheid. We are looking to interface, so would be interested in thoughts on devices that do so.

What are your thoughts and do you have any Pros and/or Cons we should consider?

My first position in the lab was in a Covid only lab. I've worked with the Liat and different variations of Cepheid. The Cepheid is my personal favorite- easy to use, easy to get sample into the cartridge, and results seem reliable. The Liat was nice because it was faster, but the one I had could only run 1 sample at a time and had a very high false positive rate to the point our medical director had us discontinue usage with it. 

Hello Erika, just wondering, why didn't you include Abbott ID Now in your option list? 

At one time the cartridges for the Roche Liat had to be refrigerated.  I don't know if this has changed since I looked at this system years ago but refrigerator space is a premium at most of my testing locations so this was a big con for me.  We use the Cepheid for all of our POC testing locations in our system.   Like Megan said, it is easy to use, cartridges are RT and there is an ability to perform several different types of testing on one platform. (The COVID and Fluvid cartridges just got FDA 501 (K) cleared for the Xpert Xpress! ) We have all of our instruments interfaced through Telcor and then into EPIC.  I have not used Spotfire but I have used the moderate complex Biofire which I thought was also an easy system to use. 

 With Cepheid being so easy to use and cost competitive, I have never seriously looked at the Spotfire. I would be hesitant to switch to the Spotfire because I have some issues in my state with some insurance plans paying for these larger panels.  If this is something you are considering, I would recommend checking your payor mix so your patient's are sticker shocked with a big bill!  

Great question on why I did not ask about the Abbott ID Now. I did not ask because our team already had previous experience with that method. Although if you have additional input on that platform, please do add your advice. We can all learn and understand more when we share!

Erika, we have been using ID Now platform for years, and recently we decided to explore other options and our team did a large scale comparison with demos and vendors interactions. The long story short, we are still with Abbott. Here are few reasons why we did not go with the other platforms, compared to ID Now (it is possible that something has changed or being resolved by the vendor): BioFire - very nice Throat (Flu, RSV, Strep and RhinoV) and NP panels, also larger panels available, BUT - the instrument is very noisy when running the test, you need different transport media for different panels, can not order just Flu or RSV during seasons. Cepheid - we have this platform in microbiology, people loves it, and it would be an easy switch, BUT you can not order RSV only, the test time is significantly longer, and the sample introduction process is a little bit complicated compared to ID Now, plus the reagent cost was a factor. COBAS Liat - it was a very good deal on the price, BUT you have to keep a refrigerator for the reagents, can not order just Flu or RSV, and the test time is significantly longer. Few words in favor of ID Now, customer service and tech support is very great!, personal approach to any problem, easy to negotiate deals or new requests, technically possible to run 3 tests with one NP swab, Covid>>reflex to Flu option, multiple instruments at a single location resolve the flaw compare to the panel style competitors. Good luck choosing your best respiratory platform, hopefully my inputs help!
