Is anyone involved with their Hospital at Home (HAH) program at your hospital?
If so, what is your process for glucose testing?
What device is used for your HAH glucose testing?
A question has been asked if an over the counter (OTC) glucose meter can be used in a HAH setting under a CLIA license?

Any information regarding glucometers and HAH would be greatly appreciated.

Our Hospital at Home program is supposed to have patients treated as inpatients, so we use a meter under the inpatient hospital CLIA.  

Hi there! We have our in-home clinicians equipped with the same (Nova StatStrip) meter that we use for inpatients.  HOWEVER, the in-home clinicians are often not present for all BG checks.  In lieu of depending upon whatever crusty old personal meter the patient might have, those HAH patients who need frequent BG checks get issued a new personal use meter from VeloCare upon admission to the HAH program, where the clinician confirms that it passes the manufacturer recommended QA with the patient.  These are for the patient to use on themselves, and the clinicians are NOT to use it. We are also clear that these device results are not to be entered into the same result field as our Nova results.  Good luck in the HAH wild west!

Editing to clarify: 
  • Clinicians are testing patients on the CLIA with an FDA-cleared hospital-grade glucose meter.
  • Patient self-testing is not subject to CLIA, and can be done with a personal use meter.

I'm based out of Canada, so not sure how helpful my response will be. (We don't have CLIA here.) We have a mixed model in place for glucose monitoring of hospital-at-home patients. Patients self-test their blood much of the time using their own glucose meters. They are encouraged to enter results into an online platform, and these results are viewable by the healthcare team. These results do not trend together with lab results or other POCT glucose results (see below).  

The majority of POCT for these patients is done by our community paramedics. They use an unconnected, hospital-approved glucose meter to test patients. They enter the results manually into our POCT middleware and the results then flow automatically into patient charts. They show up in the same place as lab tests and trend together with lab glucose results.
