Hello,

I do know that PPM is a struggle, however we are working to get ours better under control.  We are an academic facility that has multiple ambulatory clinics that are under our scope.  
  1. How do you ensure providers are trained and keeping documentation that they have been trained?
  2. With the recent ruling of a provider needing to perform the competency assessment, how are you tracking the competencies and who is performing them?
  3. Are providers entering the results themselves?  If not, how do the results get into the patient's chart?

We are doing PPM under our Laboratory oversight, so POC Coordinators qualify as technical consultants.  We document their training and recertifications on checklists.  We have old CAP pictures to assist with Fern interpretation and Wet Prep since we can't make up those specimens.  We do have the ability to make a KOH + sample, so we use that and it also helps with functions of the microscope.  We have a fair number that struggle with getting the scope focused.
For resulting, we were having providers record results on log sheets kept next to the microscopes but have moved to them putting in their own results.  We have Telcor QML and use the WebMRE functionality.  The results are put into WebMRE --> goes to Telcor QML--> goes to Epic patient chart. 

I am wondering why POCT coordinators are responsible for PPM competency because according to Joint Commission LD.04.05.01 – it says otherwise. This is the only section where it was specific that PPM Lab Director is responsible for and not the technical consultant.

Something to keep in mind - there is a difference in CLIA regs depending on which CLIA Certificate TYPE is in place for provider performed microscopic procedures.
If one is using a CLIA Certificate of PPMP (and not a Certificate of Accreditation, for example) then correct, there is no CLIA role included to use a Technical Consultant to perform the competency assessments. It has to be the CLIA LD or a qualified CLIA LD for a PPMP Certificate doing the competency assessments.
