Hello,
A few Questions for Competency at Clinic Sites...
Would anyone be willing to share what a Competency Form looks like for your facility? The form we currently use is just so squished together and not appealing. I help manage about half of our Clinic Sites (17) for their testing/competency. Initial training all happens in one location that POC is in charge of and then we have those testing track competency throughout the year at their location. We set a due date for the end of October every year and then they submit to POC. It is like pulling teeth to get them to fill in the form and even worse trying to collect the completed forms. What works for your facility?

Are you indicating that your initial training, and then all competency assessments are tracked for an employee/testing personnel using one form for each (for the year), and all of the POC tests are listed on the same form?
Just getting a handle on what you've got. Alot of us use competency forms per the test (manual or instrument, each test gets it's own form).
Your second question is completely understood (on the difficulty) for many of us here. Is the 'they submit to POC' coming from educators who are the trainers/checkoff folks, or the supervisors of the 17 clinic sites are in that role?

To simply answer the question, yes. Our initial training form includes a section for each device/kit to be signed off on (typically things that are done at each site). For those tests that may be specific to only a location or two, we have a separate training sheet that looks similar to the big training form. As far as yearly competency, each site then receives an excel grid with the performing employees name and then a row for them to check off/date when they have performed QC, a patient test and completed an accompanying quiz for each test.
Usually it is a Clinic Team Lead or Clinic Manager that should be submitting to lab as they are the final check off/signature on the form.
