Hospital at Home Test Menu

Our H@H group is wanting to grow their test menu, so I wanted to see what others are offering in this environment. They are currently working under the waived CLIA certificate of the hospital and have a glucose meter, hemocue and coaguchek. They have been "live" for a little over a year and have only performed glucoses so far.  All other lab work needed is drawn at the home and then transported to the main hospital lab to be performed.
If you have a H@H program and can answer the following for me
1-Does H@H hold their own CLIA or work under the hospital CLIA
2-Do you perform any moderate complex testing? If so, what test/devices
3-What is your test menu?
4-Do you allow centrifuges to go out in the field so specimens can already be spun down when they arrive at the hospital lab?
5-Are you CLIA, CAP or Joint Commission for accreditation?
Interested in how others are managing their H@H program

Our HAH program is closed, no longer in operation. When we brought them on, they had Nova Glucose, ISTAT CG4 and Chem 8, and Coaguchek. They did not use the POC devices except for the Nova so by the time we discontinued the program, they were only using Nova.  Everything was under our hospital Lab CAP & CLIA as the patients were considered inpatients.  I can't remember if they had centrifuges or not for the samples that needed to come back to the Main Lab for testing. I think they may have had some small ones.

It all depends on your state in terms of what type of CLIA certificate is needed. Here in NY they offer a "Health Fair" permit for mobile lab testing. In a previous life, we would do either community screening events sponsored by a town/region, or employee health screening sponsored by a specific company. Our POC lab staff would oversee the event, with phlebotomists assigned to collect and test the samples (Cholestech for cholesterol & glucose only). POC team was responsible for all the cleanup and handling of waste, the log sheets for QC testing and temperature monitoring, participant attendance sheets for test volume tracking, and ensuring any critical values were brought up to the onsite nutritionist that came with us to discuss real-time with the participant.

I just found out this week that my current lab has a separate CLIA permit for Health Fair screening that I didn't even know about. Cholesterol, glucose, and venous blood draws for PSA levels to be tested back at our core lab.
