CLIA Lab Director Site Visit Form/Checklist

Good morning,

Does anyone have a CLIA Lab Director Site Visit Form or Checklist you are willing to share? Also, does your Medical Director perform the site visit or do they delegate the visit to another leader?

We are trying to establish the process here and I would be eternally grateful for any suggestions and/or forms you can share.
