Procedures for Waived Testing

Has anyone written procedures for waived testing for inpatient and well as clinic use? I realize the package inserts are to be followed but the lab director wants lab policies for each test method. Do you follow NCCLS format? Do you break it out to follow the insert (which doesn't necessarily follow NCCLS), do you have separate procedures for QC, troubleshooting or even patient testing or resulting?

Our written procedures for inpatient/clinic use are the same (glucose, for example). Why would the procedure vary based on where the test was performed? We follow the manufacturer's insert, but also have distinct sections (equipment/materials, QC/proficiency testing, maintenance, patient testing-which includes resulting, reference ranges, limitations). The format is the same for every procedure.
