Hello all,
When auditing critical results notification, do you consider nurses as 'providers' or do you audit critical results obtained by nurses too? This is in reference to CAP COM. 30000.

When I did the auditing I would not look for documentation from nurses. Since we made a different report that can't tell the difference between a nurse and a different job description we hold everyone to the same standard.

Within our facility we track:
1) Lab to nursing hospital unit / clinic location time (may be an RN or Patient Care Provider)
2) RN to PCP (ordering clinician or current clinician on duty as in the case of a change of hospitalists from day to day
3) Overall time from Lab to PCP
Our overall time goal from lab to PCP is 40 minutes total. From lab to RN is 10, and RN's then have 30 minutes to notify the PCP. Works much better when we are able to communicate directly with the PCP.
We achieve an annual compliance rate of approximately 95-97% in achieving our stated goals. Our outliers are typically clustered with a specific location and tend to be "after-hours" communication problems.

When I do the critical POC glucose audit each month these are the items I'm looking for based on our policies:
Our institution has a multi-disciplinary (Lab, Nursing, Medical Staff, Dietary & Diabetes Education) approach to diabetic patient management. We recently updated all policies related to glucometer usage, high/low protocols etc. It seems to working very well for our small semi-rural hospital.
