I see a note in the CAP checklist under COM.01300 that says PT should be performed on waived and non-waived testing. I can handle our in house waived testing (glucose, chem8 and urine testing).Does everyone do this and what about a CLIA waived site?


I'll throw the towel in now if I have to keep up with all of those styrofoam boxes!

I will be on medical leave from December 11 to January 19, 2018. Joann O'Connor, Lifebridge POC manager and Megan Mioduszewski,POC specialist will be covering for me. Hello,


Our clinics (satellite offices) have their own separate CLIA and are not inspected by CAP.  They are inspected by Joint Commission which does not require proficiency testing for
waived testing.

Are you CAP accredited on the CLIA waived site? If yes, then you have to. You don’t have to be accredited by CAP if you just have waived tests.

CAP require PT on waived and non-waived tests. If you are CAP inspected for both waived and non-waived tests, you have to comply with CAP standards.


CLIA, TJC and COLA only require PT on non-waived tests.

After our TJC inspection last month, our pathologists decided to stop PT for waived tests. We were doing PT for every test. In 2018 we will only be performing PT for our moderately complex tests and PPMP. Our program is quite extensive because we have a large test menu and many different sites. And of course each site has to have its own CLIA. It sounds like you don't have too many tests that you would need PT for. If you are organized and set up a system/schedule, it can easily be maintained. If you feel like you're sinking and consistently overwhelmed by PT, you might want to consider accreditation by another agency.

But just to clarify that even though CAP requires PT, they don't require PT enrollment for waived tests and would accept alternative PT.

Correct. You don't have to use CAP PT samples. You can obtain them elsewhere. I think we were just using CAP samples because that's what the main lab uses.

We do not perform any PT for tests where POC is not the primary device type for that test, per CLIA.  That includes glucose, CHEM8, and urine testing at our locations that also have labs that perform those tests. So if the main lab performs that test, we do not order a kit for POC, as the lab is covering the CAP requirement that PT is performed.  A big red flag should go up if you are ever ordering more than one of any CAP survey for the same facility.


For our clinic locations we only use CLIA accreditation. CLIA does not require PT on waived tests, so we are discontinuing them all for 2018 and only ordering for non-waived.


I do agree that the language in the CAP checklist implies that their expectation is for the labs to purchase their kits for both waived and non-waived tests. However this is often misunderstood to mean that you have to do it even if you run that test in the lab - that is not so, as long as you compare the two methods twice a year or do some other type of alternative then you are fine.
