I want to redo the site inspection form for the waived sites. Would anyone be willing to share what form they use.

I would be interested in the template as well-and any list used for CAPS and/or State Inspections would be a great help

There is a site inspector checklist on the cms.gov website.


Certificate of Waiver Laboratory Project - in the body of the page there is a pdf :Questions the Surveyor uses. See link below.

At my former employer, I used this as my jumping off point for each test.

I added mine to the file cabinet. I use it as a guide for my annual site visits. The last page is photocopied. I give one copy to the Office Manager and I keep one for follow up and to review before the next annual visit.
