Hi Everyone,
We are planning on having some of our nurses be able to perform competencies on glucometer users(these are other nurses, and nursing assistants). Does anyone have a policy/procedure I can look at? Tips for implementation? Thank you.

We are in the process of implementing this as well. We are a clinic rather than a hospital, but have about 300 operators. Most of these things are still in rough draft stage ~ any input is appreciated.
We plan to give the preceptor or lead a checklist/Job aid and a sign off sheet.
For new hires, we have Stage I, II, III completed before they test patients. Mistakes still happen, but it has helped a lot. They meet with us (my supervisor or myself) for their primary training in small group (stage I). At hire and annually, all users do the online learning module and exams (Stage II). All users have a direct observation of their waived testing, but for new hires we go the extra step of having the preceptor or lead attest in writing that this person is fully competent and meets their dept (UC, FM, etc) standards on a "Stage III checklist." We have held a large POCT Fair for years, but that only got about 60% of our users. "Deputizing" a few really astute RNs has been the best idea we have come up with so far!

Thank you Mary. We plan on giving out job aids and a check off sheet that the user and preceptor sign off on. I also plan on doing an occasional email to all the preceptors that says what to focus on or tips.
Our educators must go through a Train the Trainer each year that they serve as educators. This training is with POCT staff. Waived educators must meet the approval of that unit's current educator(s) and the POCC or other lab management. If it is a non-waived platform they must have the minimum of an approved BS degree (we fall under CAP) and 2 years of experience with that testing platform prior to them being eligible to train others.
Debra, What do you use as evidence of nurses 2 years experience?
We have the same requirements here, but they have just been implemented. I have been struggling with finding a way to prove to inspectors that they have the experience (not that we have been asked, but just in case) .
Currently we have 5000+ users across many campuses/clinics (for both waived and non-waived platforms) we use nurse educators/certified observers to help maintain operators certifications. In total we have 60-65 educators, but only 20 are certified for our non-waived platforms.
Since obtaining records like this from other institutions is problematic, it unfortunately has to be on trust. I just had a thought: Maybe having a signed statement from the potential educator stating that they have minimum experience. If the lab director signs off on that as acceptable, that should satisfy the inspectors. The vast majority of my educators were ISTAT users here prior to the "promotion" to educator. I have a couple of other mod complex instruments but I handle that training since there are less that 10 people in that unit. The most recent acquisition are our refractometers. Since no one has 2 years experience on them, I and the CLS that works with me handle the training/competencies. 2020 we can pass it off to the educators in those units.

Almost all of our training is done by clinical nurse educators. I've attached the training checklist used for the glucometer. It's detailed but my goal is to have all educators covering the same information. The learner then completes the online elearn quiz and the trainer signs off electronically. The yearly competency is then done during our yearly competency fair - again mostly staffed by nurse educators. Attached is the most recent assessment form we used. This was new last year and developed to standardize the process and hold everyone accountable to the learning. We published pre-learning slides, videos and links prior to competency fair so everyone is aware of the requirements.
