Hello,

Wondering if someone in this group perform KOH prep and could share what they use for QC. I didn’t think it was required. Any info would be greatly appreciated,

Hello Yeline,


I believe you would have to make an IQCP for KOH prep testing.  Most certifying agencies send pictographs for testing. If you can show that all the pictures are available at the bench , yeast, clue cells, fungal elements, etc., this could be a component of the IQCP.


There are a large number of resources for developing an IQCP: CLIA, CAP, CDC etc.

Who is saying that KOH prep testing needs QC?  As a PPMP test, I don't believe it qualifies for IQCP.

Hi everyone,


A UMMC, we don't perform quality control on KOH. This was confirmed by our IDDL department and, I also believe that this PPM does not require IQCP. If you find out the information that supports this requirement, please let us know.

Hello!


We use a reference image collection for each PPM test that is displayed by the scope/testing area.


Our other quality indicators include PPM Proficiency Images ID (through AAFP) and training and competency modules through MTS online.


IQCP is not required (currently) because there are no commercial quality controls available.

You also can not do an IQCP (per CAP) because there is not an equivalent QC for this test. 

Ohio- All PPM testing is required to have QC. We have been informed by Ohio Department of health that photos or duplicate testing is acceptable.


It is a CLIA standard due to PPM non-waived testing.


 


CLIA does have standards for an IQCP for PPM testing.

Karen, could you please clarify, when you are saying that all PPM testing is required to have QC, are you referring to do quality control on the actual potassium hydroxide (KOH) reagent, or to maintain competency of the providers by using photos of microscopic cellular elements?

When we set this up, I called CAP and they stated that having multiple pictures of PT samples or pictures from the CLSI poct10-a2 would be a sufficient "QC".  We only have Providers performing this test outside of the Laboratory.

I was cited during my CAP inspection. Looking at pictures is a way to assess competency and not the solution. Maybe should call CAP for clarification.

Also cited for not running a lot to lot on strep or flu kits. I was told to use the QC vial from previous lot and retest with new lot. Which contradicts the standard stating not to mix kits. Any thoughts?

Hi Yeline, which state are you from? It might be a state requirement, not a CAP requirement for you. Lot to Lot on strep and flu kits is also not required, unless your specific manufacturer states that in the instructions. Which brand of strep and flu are you using?

State of Michigan. We use non-waived kits, the testing is done in our ER lab run by techs. Quidel QuickVue + (strep A) and BD Veritor (Flu A&B).
