Orders for Provider Performed Microscopy Tests

Hello all! How does your POC program handle obtaining orders from the provider for PPM tests? Paper, electronic? How do you monitor compliance?

Secondly, how do your providers result PPM? Directly into the EMR or some other way? Do they have a "drop down" choice of possible results or can they free-text?

Thank you for any feedback that you are willing to provide!

Hello All,

The providers can order and result the PPM tests directly into EMR(EPIC)- there is a drop down for the choices. We require results in patient's permanent medical record. Progress note documentation is sufficient.

For compliance, we pull reports looking at the PPM results. Also, we require the providers to submit patient's that they have performed PPM tests on and we perform chart reviews.

I hope this helps,

Tricia

Tricia,

Are you Joint Commission accredited or CAP?

We just transitioned from JC to CAP.

The providers order and perform the test in the microscope room. Results are documented in a patient log. Clinic nurses will enter the results into EPIC. EPIC has a drop down built for each of the answers.

For compliance purposes, POCT staff will collect the patient result log to perform chart reviews on a monthly basis. Providers should clean and document the microscope after each use. KOH reagent should have an opening and expiration date. Our providers need to review a learning module twice a year, including an annual direct observation of the microscopic analysis.

Pedro - who do you have do the direct observation? POC staff?

Hi Danyel,

Yes, POC staff will schedule the observation with the help of the clinic administrator to do the annual observations, preferable during their faculty meetings when all the providers are together. This process seems to work well for us.

Hi Tricia,

i am currently trying to build orders for PPM in Epic.  Would you be able to share what it looks like in Epic at your site?  I’m curious to see what the drop down looks like!

Lastly, is there a reference range included with each test?

thanks so much and any help is appreciated.  We got cited so we’re trying to get the orders built.

Anna,

Were you cited by Joint Commission or CAP? Would you share the specific citation?

Thanks!
