Excel template for Levy-Jennings

Hi POC people! Does anyone have an excel template they can share for Levy-Jennings? I need to track QC for our PT/INR meters and they are not interfaced to a web-based QM system (yet). I am seeing a shift in the Level 3 QC, although it is passing, and I need to share my analysis with the Medical Directors. Thanks in advance!

QC for Levey Jennings Template.xlsx
I do but it is for epoc gases. The first two pages (Metabolites) are for data input and the rest of the pages auto fill with Levey Jennings. Pages for HCT have both data entry and Levey Jennings. I just watched a YouTube video on how to create it and went from there.

One thing to remember with PTINR QC is this is a clot base test so tracking for trends and shifts is not necessary. As long as your result is within the range you are good.
Thanks Erika! The thing that troubles me about Roche QC Level 3 shifting from 3.0 to 3.6, while still passing, is this was seen around the time of the test strip recall last year. I think it is the lot of QC, not the strips this time, but we have the Stago as the Lab method - so this is a tricky issue.
