Primary source verification versus copy of diploma or official transcripts

For non-waived testing, I wondering do most POC departments physically verify the nurses diploma or official transcripts and make a copy to maintain in your records. Or is there any sites using primary source verification?

We use our HR for Diploma Verification. Each user must have a diploma on file. We see an electronic version prior to access. If a diploma cannot be found, we
do utilize Third Party Verification. Caution, if you are CAP and you do utilize third party verification, you must define acceptable criteria for that.

Is there anyone who is collecting diplomas of HS grads performing waived testing ONLY?
And the accreditation I'm asking about is TJC WT, not CAP.
Curious! also if anyone TJC WT inspected was asked for diplomas for testers only performing waived testing.

We require a physical copy of a diploma. Never used primary source verification, but only been doing POC for one year. Not sure about my predecessors. We don't rely on HR at all, although now I wish I could just contact them instead of pestering the users to find their diplomas! I'll have to look into that(easier said than done, I'm sure).
Oh, and Peggy, we're not collecting for TJC.
Does anyone have an example of a policy they use for third party verification that defines acceptable criteria that they would be willing to share?
Our JC surveyors have reviewed anyone performing testing include high school graduates. They have surveyed based upon job description. If the job description requires a HS diploma, one has to be on file.
Our HR department performs ALL verification of credentials. We do not allow a new employee to provide any documentation and no employee may begin employment until verification has been completed.
The HR department maintains all verified documentation, not the hiring departments.
This is the portion of our POC Policy that addresses the topic.
1. All staff will provide a copy of their degree of education, diploma or transcript, to be stored in Bronson Human Resources, employee file. Alternatively, primary source verification (PSV) is acceptable for submission to Bronson Human Resources, with the following requirements:
a. Full name and address of PSV company
b. Full name of Student
c. Date of degree
d. Degree obtained
e. College or school attended
2. The education requirement for testing personnel (TP) of Waived test methods is a minimum of high school diploma.
3. The education requirement for TP of Moderate Complexity test methods follows CMS Interpretive Guidelines:
a. MD or DO with a current medical license; OR
b. Doctoral degree in clinical laboratory science, chemical, physical or biological science; OR
c. Master's degree in medical technology, clinical laboratory, chemical, physical, or biological science; OR
d. Bachelor's degree in medical technology, clinical laboratory, chemical, physical or biological science; OR
e. Associate degree in chemical, physical or biological science or medical laboratory technology; OR
f. High school graduate or equivalent and laboratory training/experience consisting of the following:
i. Successfully completed military training of 50 or more weeks and served as a medical laboratory specialist; OR
ii. Appropriate training/experience as specified in 42CFR493.1423
We collect diploma on everyone who was hired.
Sometimes they are hired to work in a department
that does waived tests only but later on
they transfer to a unit that does non-waived.
It is a lot easier for HR because they do not have to ask
the person to bring a copy of their diploma when they transfer.
Our HR keeps all the diplomas, transcript or PSV.
Our HR rep was present when TJC was here last year and went over
the tester’s qualifications, education, competency records, etc.
So they know what lab has to go through after a tracer.
Julia, TJC now ask for diploma, transcript or proof of education by primary source verification.
It is in the CLIA regs so they have to enforce.

Our new POC director wants me to devise a form that the educator or manager fills out to attest that they have verified the educational requirements for every new POC user along with the color blind test that she can then sign off on. Does anyone have something like this?
I'm not collecting diploma for Waived testing, but only for Non-Waived.
For most employees, our HR got their education verified. But somehow, some of them are not verified (i don't know why), I then started to collect a physical copy. it was hard at the beginning, but after a while, the managers have it on their list to require whenever they have new hired ones.
