the POC program at our facility just cutover to Epic/Beaker hospital wide.  We are having issues with results getting hung up QML exceptions possible due to CSN numbers??? When looking a patient up in Nova net by medical record number I can view multiple CSN numbers associated with one patient-these are generated per "encounter".  I have hundreds of patient results in my exception list and not sure how to resolve.

We are not using QML but have been on EPIC since Feb. 2016.  You will need to always use the ADMISSION CSN from the list of Encounters.  This CSN should be the number printed and/or barcoded on your patient armbands.

The  CSN number attached to admission is usually highlighted in green. This is the CSN number that needs to be used to send the results.

Yes, each patient has one MRN but can have multiple CSN depending on inpatient admission, date of OP visit, physician office visit, etc.  But there should be only one "admission" CSN for an inpatient.  You can verify this using EPIC Patient Station function.  Unfortunately wristbands can be printed referencing any CSN (unless there are user restrictions in place).  If represented in a wristband barcode, that information is scanned by the meter-analyzer, with the test record ending up in the Telcor exception file.  I suspect that is what's happening in your situation.  I know, it happened to me years ago.


Report this to the implementation team.  Until the admission CSN is represented on your patient wristbands, you'll continue to receive lots of test records associated with CSNs representing other patient activity.


Consult with Nova because they can verify what pieces of patient data are being received in NovaNet.  Telcor needs to be involved too for the same reason. And consult the LIS team as there is a correct CSN - MRN combination at play there too.


Otherwise, left as is, the problem continues, you're stuck with lots of result exceptions, and your users start to think "your" meters-analyzers are at fault.


Workaround:


EPIC Patient Station function.  Search for the patient by MRN - CSN - name. Look for the admission CSN.  Alternate: search for the patient using Telcor ADT function and look for the inpatient admission MRN - CSN combination.  Change the Telcor exception sample ID to the admission CSN, and resend that record.

This happened to us @ "go live" as well.  what a mess it was.  For those of us that like things neat it was disastrous.  We went "live" in November 2016.  Yes, the admission csn is the key.   We have RALS/Beaker/EPIC. Good LuckSmile

Involve implementation and teach to use the “armband CSN”


Novanet will have your list of patients


Telcor ADT – search by MRN – find the CSN for correct date – manipulated the date/time in the ADT if needed.  Go back in to exceptions – change the sample id
to the correct one found under ADT MRN search and send to chart.


 


Do you have many overrides on the meters?  Are operators seeing the display of patient ID on the meter screen?  They should if using the “right” armband CSN.


 


Hang in there – it gets easier but it is a process and takes time.

Does anyone know how to look up a "HAR" number in Epic?  Can this be done in patient station?


 


Does anyone use Cornerstone and Telcor for POC?  If yes, does your facility for an eLearning interface with Telcor.   

We can type "HAR.XXXXXX" or "CSN.XXXXX" in the MRN field to pull them up.  Caution however...I have used three different EPIC systems and they all are set up a bit differently.  Give it a try!
