Quality Assurance (QA) Policy

Good evening,
A Ga Dept Health surveyor came and inspected one of our newly opened clinics a couple of weeks ago. One of the recommendations: Put a Quality Assurance policy in place. I was not at the meeting, but my hospital lab manager followed up with me, the Technical Consultant, to give me my assignment.
Am reading a piece on CLIA and Quality Assurance (AAFP) and find myself feeling a bit overwhelmed looking at the ten standards. Does anyone have a template he or she could share, resources to share, or just words of encouragement to offer.

The article explains how it should be done. Make sure your policy explains each one.
When you do the monitor, for each location, result should be in compliance or not.
If not you have to have remedial action.

The items on the template are tasks I do now, but I do not have a policy about them - yet.
One question: the patient log/tracer. Could you elaborate?

Clia and quality assurance, i have done monthly audits on the units for the POCT devices
and reagents. Unit nurse managers and educators were involved if complaint or non-complaint, documentation and reports were done and huddles were also apart of the audit.
Rules and policy would be apart of your hospital patient identification poilicy. I worked 19 years for quest diagnostics we were CAP ACCREDITED.

This is form I use to monitor tests done in the clinic.
Who is the patient? Is the MRN recorded on the log? Was there a doctors order on the chart? Was the result logged in the Patient log? Is is it the same result reported in the EMR? Who did the test? Make sure they have a record of competency. Were the initials logged in the patient log? . If result is critical, was the physician notified? Were all the internal QC recorded? Was the QC done on the Test kit?
