Waived competency testing

Do you give written tests (short answer/multiple choice) annually with your waived test competencies? We are trying to figure out if we have to do written tests every year with skills competency or if using them just at initial training is sufficient. We are not accredited (no CAP or TJC), we have our license through the state department of health. Right now we are waived testing only, no moderate (I know things are different with mod complexity).

I do quizzes annually only. We only have waived testing and are not accredited either. I do most of the initial training so I am "quizzing" them as I train them.

For our waived and non-waived tests, each tester has to complete required modules in Healthstream  and take an exam annually. You would be surprised how some testers do not remember what they read. So when they come to the lab station, they also get quizzed orally.


Most testers only do waived test (  glucose ).   So at the lab station they get to walk us through the finger prep and how to do the test.  They get to test QC and an unknown sample. I think the  preanalytical step is very important in getting a good blood sample, so we do not take that step lightly.


Our clinic personnel also have access to Healthstream so they have to complete all the modules for the waived tests they do, that includes kit tests.  All the kits they used are standardized so everyone use the same kit, same modules, same test.  It also helps if they rotate to other clinics.


 

For waived tests, how to competency assess is up to the lab to decide.  Taking a quiz is not a requirement, but recommended for obvious reason.   Whatever you decide to do, make sure it is written down in your policy. 

Waived testing competency follow my CLIA guidelines have a CMS license. i do initial training, 6 months quiz assestment then annual competency. Folks have a tendency to forget regulatory guidelines so it would'nt hurt to implement good laboratory practices.


we were a Hospital 1 year ago; now we have transition into a urgent care, communuity health village futher healthcare.

 


 


For waived testing, you are only required to use 2 of the 6 elements of competency. If you use more it is better.


Exam and doing QC is the easiest to achieve.

A general comment going back to Don's 'it's up to the lab' comment. I think this is especially important to keep in mind in reading any comment about training, competency, competency assessment. A lot of us totally go by accreditation standards (meeting them) but that doesn't mean each of us are held to the same 'compliance' where we work, depending on which CLIA Certificate we use, etc.


 


On a listserve it may be difficult for new folks to POC compliance to sort out what one 'has to' do, 'should do'.. in terms of sorting out our comments. As always 'your mileage may vary' from site to site, institution to institution, state to state. 


For those who have the nova stat glucose meter and use RALS for monitoring annual competency, what is your practice?


We auto-recertify if the test operator successfully performs both levels of QC and 1 patient within 90 days of their due certification date.


They also are assigned an annual quiz through e-learning, which they must pass.


We haven’t been printing anything from RALS, annual competency assessment is all electronic in RALS.


 


Is anyone else using the auto-recertification process?  Do you print anything from RALS to have placed in each test operator’s personnel file?

For all of our waived testing, we do initial training, 6th month, and annual competencies - all given by our education department. The RNs all over the hospital needed to be comp-ed on a lot of other things anyway, so we made an arrangement with our Educ department to give it out as part of their packet. 


 


 

We are CAP accredited and require an elearn every year, even for waived testing. Some of those are just a quiz and some include a quiz and a PPT or video.


We have also always had waived testing at our competency fair. Depending on issues that arise over the year, we may focus on a different aspect of the test - instrument operation, QC, sample collection, troubleshooting, etc. This past year, we collected Survey Monkey data on each operator and based off that we know better what struggles staff are having and what's going well. I've found a couple tests that I may just change to an elearn and stop having that station at comp fair.
