POCT Accreditation structure (waived vs nonwaived)

I am curious as to how other Hospital and Clinic based POCT programs are set up for regulatory accreditation. I have oversight of 23 clinics that perform only waived testing. All are accredited by Joint Commission. I also oversee the waived and Moderate POCT in our hospital. The waived testing (just glucose and pH) are accredited by Joint Commission but the Moderate (iSTAT, Hemochron and HMS) are accredited by CAP. It has been a pretty easy distinction up until yesterday when the Perfusion department want to perform waived Hemoglobin testing. I am torn with leaving the regulatory distinction by department vs test CLIA classification. Anyone else have a similar setup?

We have all our hospital POCT under CAP, whether waived or non-waived. For the clinics, they all have their own waived or PPMP CLIA certificates. Those that are owned by the hospitals are also subject to TJC inspection. We also have a few sites now that are COLA accredited.
I would think that it would be simpler and easier to manage to have the POCT regulatory distinction by department rather than CLIA classification.
