Competency Requirements for Waived/Non-waived Testing

Hello,
I am trying to find out what some labs do for competencies. For waived testing such as Accuchek glucose testing, does it require direct observation for annual competency? We currently only have a quiz we complete online but it does not indicate direct observation is done or monitoring of problem solving, etc.
Also, if you require to perform an audit for nurse operators, would you pick a select number per year to audit or attempt to complete all nursing audits before the year ends?

It can be done, although I don't have buy-in from my parent hospital yet (old school). Here is my plan to cover the 6 elements:
#1: Enter the date for the direct observation and the observer's name in your middleware or excel sheet.
#2: Have them run a fake patient or review the patient results they ran.
#3: Have them run a QC.
#4: Same as #1 above.
#5: Have them run a fake patient or proficiency testing sample.
#6: Have them take a quiz.
This is what we do for all of our Non-waived tests that are interfaced to RALs:
1. User is assigned the module in Healthstream. This module includes a video, information about the test and includes about 20 questions which cover all of the 6 elements required by CAP.
* This module is assigned by our Education dept to all nurses at the same time. Each nurse has 3 months to complete. If not completed by the due date, our HR department places the nurse on unpaid Leave of Absence.
* At the end of the module, the nurse must print an Attestation page which is their Observation form. They must be Observed performing a control or a patient. Once the paper is filled out, it is returned to me. I then go into RALs and update their annual competency date. If the Observation is not returned by their expiration date, they are locked out from using the instrument.
We have 100% compliance every year because of the consequences put into place for non-compliance.

Who does your observation and what qualifiers does the Observer have to meet before they can attest to the competency of another?

We don't have that many, so I am able to verify each competency myself.
