COM-209 form

Is this form that is filled out, signed by the medical director and kept at the facility? Or do you send it into CLIA?

The CMS Form 209 only needs to be sent to CLIA if there is a survey or changes to key personnel such as the laboratory director and technical consultant/supervisor AND this is a Certificate of Compliance laboratory. I am not familiar with the COM-209 form but I am guessing this is a CAP (or other accreditation agency) form. It should not be sent to CLIA.
