CAP Proficiency Testing - not submitted

Hello, all! I am looking for your expert advice on how to proceed. We have CAP proficiency testing for the i-STAT ACT testing in our Cath Lab. Our lab failed to SUBMIT to CAP on the due date. May I ask for advice as to the corrective actions we need to take? 

What testing should we do, and should we notify/submit something else to CAP, after-the-fact?

Thank you in advance. Would really appreciate any input.

Hi Mary,
If you still have the CAP material or it was performed and just not submitted -Perform the test and manually assess the results based on the CAP limitations.  CAP will list it as a failure so during the inspection process you'll have to produce documentation of corrective action.

Write a disposition explaining what happened and have the Lab Director sign off on it. (See the attached form we use for a survey failure, <10 labs reporting, etc.

If the results failed then you can purchase another assayed material to perform - Medical Laboratory Evaluation, Main Standards, CAP material if still available.

Hi Mary. Here is what I have done in the past. We would wait until CAP has posted results and compare the results we didn't submit to be sure we would have passed. We also developed a process improvement plan to be sure this wouldn't happen again. We added due dates to 2 techs calendars 3 days before the actual due date to ensure it doesn't get missed. Then I had the lab director sign off on our failed proficiency form. I use API and they do not require any submission after the fact. Good luck!

Yes, definitely have a written plan to show inspectors and documentation that your lab is following it.
