Laboratory Director Signature Page

I oversee POC testing in 30+ physician offices. Each office has a binder with hospital POC policies, procedures for each test system, documentation logs, etc. Before I took this position, the process put into place was for the laboratory director of the facility to sign a "signature page" yearly that is placed in the front of the binder. This signature page broadly lists the test systems used (does not specify manufacturer), states the location of the clinic, and has a signature line for the physician. I'm not sure what requirement this is fulfilling, if any. All testing is waived. I guess my questions is: is the laboratory director signature required on each individual procedure written or is this general signature sheet sufficient for approval of the policies and procedures contained within the binder?

The rule is to have them review and sign the statement for all tests performed under them every 3 years. I usually send out every other year just in case I happen to miss a year, we still won't be out of compliance. All policies are electronic now so I'll email the physician with a link to all the clinic policies and the following statement to sign.
