iSTAT Glucose AMR validation to 700mg/dL

Is anyone aware of a third party material that can validate glucose up to (or close to) the reportable range of 700 on the iSTAT? Currently we are using the Tri-control cal ver set and the highest level is 567. A department is inquiring about widening our reportable range.

If you are using patient comparisons, how many would you use in such a validation study?

Thanks for any advice!

Thanks, Alma. Sadly, they only had an option up to about 600.

I honestly would go with your current AMR product and report to 700.  There are a number of iSTAT analytes that do not go all way to top of range with APOC products. Who accredits your lab?  Accred will accept your validation as long as signed off by CLIA medical director.  the TRI control varies a bit from lot to lot but the current for our site is mean of 571 but acceptable range is 485-657. I used that and report to 700 for AMR. That said I also do a glucose meter to iSTAT to main lab compare at least every 6 months and always have some high values in that from DKA patients coming ED/ICU.  in doing this i see some >700 that main lab is more than 700.
other ideas:
-What does the main lab do for validation of chem analyzers - perhaps you could use their product and compare with main lab at same time (I do realize the sample matrix whole blood to serum/plasma issue - but in my experience they usually work).
- You can also take some whole blood heparin tubes and spike with glucose - run on iSTAT and in main lab - in duplicate if desire. My glucose meter manuf. supplied me with spiking solution.

Hi Everyone:

While it is good to be able to report the result of Glucose up to 700mg/dL, but if that product is not available and you have something that can make it to 600mg/dL, I will take that. After all what is the difference between reporting >600 mg/dL or 700mg/dL in the management of the patient on hyperglycemia?

At what level of Glucose result will you initiate Hyperglycemia protocol in your organization? Will a result of 600mg/dL or >600mg/dL changes the management of your patient? How many patient will ever get a result of 700mg/dL or >600ng/dL in your organization? The interventions that the nursing protocol for Hyperglycemia will initiate probably will prevent your patient from getting a result of 700mg/dL if they are still alive at this level.

Sincerely,
Alma
