PTINR Testing at Stroke Centers

Are any of you using a POC PTINR for your CVA patients that present to the ED? If so, what are you using? And, who performs the testing? Our ED physicians are wanting a POC test because we can't seem to get our TAT to a satisfactory number and I'm just trying to gather information. Any and all responses are welcome and I'd be grateful for them.

We aren't using POC coagulation in our ED. I'm curious as to what is the ED's expectation of a satisfactory TAT?

Tim, 
A satisfactory TAT is 30 minutes or less for our Code CVAs. Not at all unreasonable. We just can't seem to find a process that gets it done consistently in that amount of time. 

As far as I am aware, the only CLIA approved INR test that is not for monitoring Warfarin therapy is the Hemochron INR test. We use this at our freestanding ED/UC sites, as well as ED STAT lab sites. None of our other hospitals/EDs do POC INR.

Also, this test is moderate complexity, not waived. 

Thank you Kathleen! Are you using the Sig Elite for that? Do they have the INR on the GEM Hemochron 100s?

Yes, we are using the Sig Elite. Sorry, I don't know if it's available on the 100.
