Competency Requirement for Physician Perform Waived POC Testing

Dear all,
Could you please share your policy regarding competency assessment requirements for physicians performing waived point-of-care testing? CAP and TJC checklists indicate that competency assessment is not required for this group. However, California state regulations do not appear to differentiate exemptions for physicians. It only classifies as testing personnel, and all testing personnel may be required to meet competency requirements for both waived and nonwaived testing.
At our hospital, ED physicians perform Hemaprompt testing, and we are currently reviewing and updating our competency assessment policy. Your guidance and any examples of your current practice would be greatly appreciated

Hi Danni,
  We are a large health system where physicians also perform this test.  We opt to have physicians participate in an annual electronic learning module which walks them through the procedure and also includes a quiz where they have to interpret a positive, a negative and an invalid result.

Thank you for your feedback. We previously followed the same process for competency assessments. However, about three years ago, we updated our waived testing competency assessments to align with California Title 17 requirements. These regulations specify that waived testing competency must include five of the six elements, including direct observation of patient testing. As a result, competency assessments must now be conducted in person. It is very inconvenience for doctors. So, I am trying to review the process. 

Oh wow.  Yes, I remember those days when all regulatory required those elements even for waived testing.  Certainly a struggle with physician users.  If you can't achieve compliance under your oversight, I would consider spinning it off to a separate CLIA and let the physicians themselves run and maintain their own program.  Or withdraw the test altogether at the POC.
