Issue: Has anyone had an issue with the glucometer scanning a patient's armband ID (linear barcode), changed one of the numbers to another digit, that number matched another patient's ID in the system and posted the result to the wrong patient. Since the number that was scanned "incorrectly" matched an active patient in the system, the "Patient not Found" warning did not populate.
Questions: Could someone explain their process for PPID using these 3 systems? Do you scan a linear barcode vs. QR? What patient ID (MR#/Account) is scanned from the armband? Does your facility use CSN#'s and they are issued in numerical order? How do you verify that all patient ID's that do not hit the exceptions report for ID are correct?

We are using the same devices and LIS. Staff scan the CSN# which is a linear barcode on the armband. This miss-scanning happens sporadically, but the process for nursing staff is to rescan or change the armband if they are not getting positive patient ID at the meter. If they still get an invalid ID message, they are to fill out a downtime form. I am not sure why the miss-scans occur but could be due to low lighting and minor imperfections in the barcode on the wristband. Make sure your armband printers are on the darkest settings as sometimes they print faded and can lead to scanning issues.
If you use Nova Net, there is a positive patient ID functionality that you can send ADT from Telcor to the meters and have staff verify name and dob after scanning chart label. Our facility doesn't seem to have any issues with miss-scans, so I assume it's a function of the wristband rather than the meters. We're using the older model of the glucometers though, do you have the new model?

As far as I know, we have never had an occurrence like that. This would be extremely rare for us. We have seen an incorrect ID match a patient in another facility, but Telcor would flag any testing that doesn't match up with the patient's admitting facility
