DCA Vantage A1C - billing?

Our Diabetes Education center uses the DCA Vantage for Hemoglobin A1C testing. They haven't previously billed for this test, but have inquired about the potential to start. Does anyone bill for A1C testing on the DCA Vantage at an outpatient facility? If so, could you provide any information on CPT codes, pricing, etc.?
