Manual Patient Result Forms

Hello All!  Looking for an effective way to manage manual POC test results at our clinic.  Not all the areas perform the same tests, but there is some overlap.  The final process is to scan the forms into the EHR.  I'm trying to standardize and wasn't sure if one form with all tests available, or a seperate form for each test?  Anyone willing to share some of theirs?  Feel free to send offline.  Thanks so much!!

Our Clinitek Status analyzers are not interfaced. Operators print out the test results and tape them into a log book. When I visit sites (appx. once/month), I photocopy the sheets where the results are taped. Unfortunately, not all offices are as meticulous as some, and the tape will be placed across information, and the adhesive reacts with the printout, causing fading. While it may not come through on the photocopy, it's usually still legible. If it is no longer legible, I retrieve the results from the Clinitek. I fill in the missing information on the photocopy.
