Unable to position

I know this was asked several months ago, I wasn't haven't any issues then.  But now I am with Lot 212D170900243 iSTAT cartridges.  The therapists are getting codes 31 and 34-unable to position sample.


The expiration date on the cartridges are 11/28/2017.


Anyone else having issues?

We are also noting more code 31 and 34 now and earlier this year around March. We have not used that lot #, sorry to say, so I don't think it's just a lot #
issue.

Is this G3 or CG4?  We get a lot of these but we do so many it's difficult to judge what are operator error and what may be cartridge issues.  However, we had to pull a couple of hundred creatinine cartridges that were giving unable to position errors.  I never heard back from Abbott about it so they are still in my fridge.  Not the blue style but was still an issue.  I suspected a fault in the length of the channel because the operators said it would work if slightly overfilled.  Ivy

We think is G3 since we're seeing it in areas that don't use CG4, and seems to be widespread, not just one location, which would more likely be G3 as they're used hospital wide.
However I do see a few in Imaging/Creatinine areas. And I also recently had an iSTAT that would not run a PT/INR at all, gave code 31 every time, while those same cartridges worked fine in the iSTAT right beside it. It's perplexing.

I've been seeing more code 31s with the Chem8 cartridge (about 7% of tests vs. around 1% on previous lots). It seemed to be the lot because it was happening across operators and devices. I sent the lot (H17072) in to Abbott for investigation but haven't heard back yet. I feel like there's been more than normal since that lot, but not as bad. I've run the conditioning cartridge but I don't think that's made a difference.

I have been seeing more code 31's on Chem8 lot H17148- the lot previous to H17072.  This was during revalidation of ER and Cath Lab i-Stats for a new facility.  I was the only operator but I have done my share of validations, so I don't think it is operator error.  This was occurring on multiple devices as well.

Does everyone know that under most QC code results you can see what cartridge was run based on the "panel code"? For example a G3 = 0C, CG4 = 0A, CG8 = 05, CHM8 = 04, PT = 22.  It's an easier way to filter for review over multiple locations.  Ivy

We are also seeing a significant increase in codes 31 and 34(unable to position), specifically in the Creatinine cartridge since January. We have ruled out operators and analyzers. We are working with Abbott. It would be helpful if any other I-Stat users, if you have not already done so,  who are experiencing this same increase in codes,  contact Abbott hotline and open an issue.

I contacted Abbott yesterday.  They told me to use the ceramic (conditioning) cartridge. They are also suppose to send new lot number of cartridges (G3+) to see if getting same errors..

Abbott agreed to credit the unused creatinine cartridges from the last lot# I had trouble with.  Nothing for those that failed with the code 31 and 43 that I reported. Ivy
