Fern Testing

We are looking into removing the Amnisure test (due to non-coverage by multiple insurers) and reverting back to Fern Testing. In the past this was only performed by Providers, now we are looking at testing with RNs. Does anyone have experience with RNs performing Fern Testing? What do you use as a control?

I am not sure if RN's are authorized to perform Fern testing. I thought PPMP was done by a Physician, Midlevel practitioner, or Dentist.

I thought so too. I called CAP and they stated that the fern test could be done as an RN and treated as a moderately complex test under the regular POCT inspection section.
We have RNs perform fern testing here and have never used PPMP. I treat fern testing as a moderately complex test. For part of their training/competency I have photographs with ferning and without (from CAP, API, or the internet) which they identify. They also must be observed performing a patient test. Since there is no commercially available QC, I wrote a policy that states once per month a patient slide would be read by two different operators. They each record if ferning is present or absent. I have a form for them to record their results on. If they agree, great. If they disagree, a third person needs to look at the slide and record their answer. The outlier operator needs re-education. We are inspected by The Joint Commission. I have never had an issue with RNs performing fern testing. My QC log/policy was started in August 2016. We are being inspected end of this year or beginning of next year.

It depends on the type of CLIA you have. PPMP does not allow RN’s but if you are in the hospital and your RN’s are performing Ferning
Under your Certificate of Accreditation it is ok.

Check with your state also. In the state of Pennsylvania Fern testing is not considered in their scope of practice for an RN.....unless there has been a change in the last 5 years I'm not aware of. Any other Pa people under the same impression? I can't put my hands on where I read this right now.
