Waived whole blood glucose

  CAP no longer manufactures a proficiency testing for whole blood glucose. What


is everyone using as an alternative assessment?  Because it is waived testing, a


PT is not required.  We do a whole blood glucose cross-check (WBGQ).  Is this


sufficient as an alternative assessment?  What do you think?Kiss

We use the WBGQ.  It was recommended byCAP.

I chose to do a record review instead. Every 6 months we find 20 patients run on both the meter and a sample sent to the lab within 10 minutes of each other on
20 different meters. 95% of all values must match within 20%. We preferred this over the cross check because it evaluates a large number of meters and operators and also serves as a lab to POC comparison. And it’s free!

Why do you need to do an alternate assessment if PT is not required? Just for your own satisfaction?

On the Laboratory Accreditation Program-Laboratory Menu Form, there is a column that is marked with a "Y" that states Alternative Assessment Required.  The analyte is Glucose, whole blood, waived (glucose meter), POCT.  I am assuming that the Cross-check is sufficient...

Lara- Does your lab run whole blood glucose?  I like the idea, but we rarely do WB in the main lab.

We also use the WBGQ as an alternative assessment. We can do 30 devices with each mailing, so with two mailings per year we can cover more than 50% of our devices.


No, we’re comparing the whole blood to a plasma sample.

We have not done WBG PT samples for 17 years when I took over POC.


It saved us time and money.


Our  waived testing was under  TJC  and now we are TJC even the  non-waived tests.  


TJC does not require PT testing on waived tests. CLIA don’t require it.


Unless your State regulations  require PT on waived tests, I see no reason to do them.


But I do comparison testing  between Glucose meter  and  main lab instrument glucose.


Even though it is not required. I use data from  the computer. 


I end up with at least 50 or more  data points per 6 months. Statistical results I get  are very good.  


Comparison testing makes more sense to me to check meter/instrument accuracy. .


The WGBQ program is sufficient as an alternative assessment, it's what we use also. There was a CAP Q&A webinar about it a couple years ago. I tried searching CAP's site for it but since they've updated the link won't work...Let me know if you want me to scan in the questions for you and I can email it!

CAP labs- can use it as an alternative PT assessment, that is what it was intended for.  You technically run all the meters yourself as it is a cross check NOT a Proficiency test but I have nursing staff perform the testing.  I use this as an informal teaching/competency assessment moment.

CAP has  a PT called HCC-2, a waived combination. You can use for Glucose, Dipstick, HCG and Hemocue.


Check it out.


They have 4 shipments per year.  Two are for Glucose 3 samples per shipment and the other two for misc POCT.
