iCa for i-Stat

We do not perform ionized calcium testing at our hospital, therefore we have no way to correlate the result. Has anyone else had this issue, if so, how did you handle it.

Correlated with our reference lab
For our ACT’s that we do on our iSTATS, since the lab does not perform Activated Clotting Times, we do an iSTAT to iSTAT comparison. We have 2 machines and we do a patient comparison to each other.

I had the exact same issue recently. Needed to revalidate my Chem8+ cartridges. Since no instruments in my system analyses iCa, I have gotten in contact with Mayo for iCa correlations. Due to the COVID19, they have suspended sending correlation specimens out for validations. What you will have to do is collect however many patients you plan on analyzing for your iCa validation (typically 20) and make sure they stay capped and spun down within an hour of the collection time. Specimen requirement for iCa as per Abbott is to maintain anaerobic conditions. So ideally, drawing 2 tubes from the same patient would be ideal if during your analysis on the iStat, you happen to open the tube. Do try to get in contact with your reference lab and work out to see if you can create billing codes for the validation specimens.

You don't need to "correlate" a test to another testing platform if you don't have another platform that performs that same test within your facility.
You do need to validate the results as accurate and precise. This can be done with the Abbott Tri-Control Cal/Ver for the iSTAT iCa.
If you have multiple platforms of the same type (multiple iSTATs for example) you do need to perform a "instrument to instrument" comparison.
Jeremy, you beat me to this response. The requirement is to prove accuracy, and that can be done in many different ways. For instance, with the cal ver kit for i-STAT. It is NOT required to compare to a reference method. I have verified this with CMS, CAP, and COLA.
Cindy, We do not compare I-STAT iCa to reference lab or another method. We do not have another method in house.
We are now TJC inspected and the inspector told us that there is no need to compare I-STAT to I-STAT since it is the same method. I-STATs are readers. Testin g happens in the cartridge.
WE just do I-STAT to Main Lab comparison.
