Testing personnel and repeats for Hemochron APTT and EPOC creatinine

Does anyone utilize the Hemochron for APTTS and/or the EPOC for creatinine?

If so, could you please let me know who draws and performs the testing? We are currently discussing this, and our administrators have differing opinions.

Also, do you perform repeat testing on the main lab analyzers for any of the APTT or creatinine specimens?

Hi Tracey,

We use the EPOC for creatinine in our Radiology department.  For the ED Radiology either the Rad Tech will draw or the nurse will draw when starting the IV.  In the outpatient area, the Rad Tech draws.  We do not generally perform repeat testing unless it is also ordered (ex. an ED patient with orders for a CMP/BMP also would have the creatinine performed in the lab also); however, if the result is questionable, they will repeat on the EPOC a second time, and if the result is still in question, they are advised to order a stat lab creatinine to verify.  Although, I haven't run into this issue often/if at all.  I hope this helps!

Thanks for replying. Does the person drawing the sample run the test, or does a tech run them? This is where we are at a standstill.

The Rad Tech runs the test in both our ED CT and in outpatient Radiology.  So, most of the time, the person drawing is running the test with the exception of if Radiology obtains a sample from a nurse draw for an IV start in the ED.   We don't have our ED nurses run the EPOC as the test is for the Radiology department.  Hope this helps and good luck!

We did an evaluation of the Hemochron APTTs and found it to be a good test. However we are not using it right now. The EPOC, we use for creatinine in our oncology suit. Based on our total error evaluation we decided to have results over 3.5 mg/dl confirmed with a lab test. We have a lab tech in oncology already so they run the test. We could train up  an RN to perform either test though.

Thanks for all of the information, Thomas. I'll have to work on our error evaluation before we go live. I think it is more practical to have a tech or RN that is already in the patient care area to perform the test, so I'm glad to see that you and Jacqueline are doing that.
