ISTAT during CPB

Do any of you use the ISTAT in Perfusion for CPB?
Our perfusion department would like to switch from the Rapidpoint to the ISTAT CG8.

Thank you,

Sylvia

We use it.

Kammy any issues noted by the perfusionist or you?
In our Perfusion department they may run several ABGs during a case and I was just wondering if doing
five CG8 cartridges (same Sample ID) and the docking the ISTAT causes problems.

The only issue we see is the H&H do not even closely resemble the actual patient results.  I have wireless ISTATs so we don't have issues with them uploading either.  Our perfusionist choose to use a printer and print their results.  So they have them as they work.  I haven't seen or heard of any other problems.

We were using it in our OR, but with the newest product notification letter coming out and their Instructions for Use being updated today to state that for patients with an O2 >100, the PCO2 can be increased by approximately 1.5, our Medical Director pulled the cartridge from being used.  We are now exploring other methods for blood gas testing for our OR and ECMO patients.

We received that letter also...we are aware of it but are moving to the Siemens EPOC and away from ISTAT.  And we live at an elevation where we don't see very elevated O2's so the director isn't concerned with it.

I use i-Stat during CPB.  The H&H did not match very well until I used CPB mode in the i-Stat.  correlations were much better with CPB mode.  The perfusionist didn't seemed very concerned with the recent pCO2-pO2 issue.  Everyone like them and we have no plans to change.

Our perfusionists do not like the iStat and have been asking us to get rid of them for a few years now.  So with this latest pCO2/ O2 issue we are finally moving in that direction.

Do any of you have an issue with the upper range of the PO2?
I know abbott stated a reportable range of 5-800 but we have verified 34-466. (use Abbott 5 level linearity kits)
Do you add another level to get a higher upper range?

Thanks, Sylvia

Sylvia,

I've used the Eurotrol CueSee hyperbaric and Hypoxic for this exact reason for nearly 10 years: to extend out the AMR that my RT's wanted but the Abbott Cal/ver's didn't reach.

  • Due to the issues that Abbott has had of recent we are leaving iSTAT and moving to the Siemens EPOC this year.
