Benchmarking-threshold target rates for MRN errors

Hello,
Would anyone be willing to share what their institution's threshold target rate is for MRN errors for point of care testing.  Our current rate is 0.01%, but we don't have any historical data as to where this number came from.  Does anyone else track this as a quality metric?

We monitor this every month and publish it to the facilities and leadership, with our current system we use a 0.3% threshold.  We are moving from POC middleware Pweb to TelCor next year and I hope to lock down testing better with the new system to improve this significantly.   Its a daily task addressing this.

We track this monthly at 5 of our hospitals.  I don't know how you are calculating your %, but I must say, your threshold sounds wonderful! Kudos! 
At our largest hospital, 1100 inpatient beds/4,300 glucose operators/50,000 glucose tests per month, we saw an average of 102 per month through this last fiscal year (July 1, 2019 - June 30, 2020). Of those we had an average of 18% not corrected each month.  The size of this one hospital does offer unique challenges for me to communicate, isolate, and correct these types of errors. I am hopeful that once we can focus on issues other than COVID-19, that I will be able to spend time with our clinical leadership addressing these patient ID errors. 


Side note: we also had a significant LIS build/go live and brought a new hospital onboard in this last FY. We never have a lack of active projects and new builds in our system. 






This is an ongoing quality monitor for us. Target is < 1.0% (not for errors, but for errors that we can't fix and get to the chart). We have maintained that for two years now, and we are always looking for ways to reduce the errors from the operator end to reduce the amount of work we have to do to get the result to the chart.
