POCT Urine Drug Screens

Looking for guidance on how your organization performs POCT Urine drug screens. We have had multiple requests for this testing in our clinics that have patients on Pain Contracts. We are wanting a CLIA waived methodology, but those types would require manual entry of 15+ results into the EMR for every patient. We worry about human error with reporting results into the EMR and also interpreting results. The area requesting this testing also perform other POCTs (strep antigen + urine pregnancy) where a line at the testing section means positive. UDS tests is the opposite (absence of a line is positive). We also see that confirmatory testing must be performed on all POCT positive results - is there a workflow for ensuring this occurs? what about billing the patient for the POCT and any required confirmatory testing? What if the confirmatory test doesn't match the POCT result? Have you had any issues with QC or Proficiency testing? Looking for any guidance, words of wisdom, or compliance struggles with offering this test. Thank you!!

"UDS tests is the opposite (absence of a line is positive)."
- We use "MedTox" with readers in the LAB that are interfaced to avoid errors in reading for this reason. We do not do POC UDS testing for the same reason.
