Dear Point of Care Coordinators,
I am gathering information regarding the collaboration between Point of Care Testing programs and Emergency Management teams in healthcare settings. I would like to invite you to complete a short survey by clicking on the following link or copy/paste into your browser.

The survey should take less than 5 minutes to complete. Identifying your facility is optional. I am interested in learning if your Point of Care program has an Emergency Preparedness Plan or could benefit from guidance in developing one.
Please do not hesitate to contact me at elliba3@upmc.edu should you have any questions and feel free to share this with any peers you believe may participate. Thank you in advance for your participation!
